Our Transition Service supports young people aged 17.5 to 25 with severe and enduring learning disabilities—and often physical disabilities—as they move from childhood into adulthood. We work closely with their families to ensure this transition is as smooth, personalised, and empowering as possible.
This period of life can be challenging for anyone, and even more so for young people with disabilities. That’s why our team uses a person-centred approach, guided by the Preparing for Adulthood framework. We respect every young person’s right to make decisions, and we help them stay in control of the changes ahead.
We’re looking for a passionate, proactive, and motivated registered Social Worker to join us—someone who’s committed to helping young people and their families shape their futures with clarity and confidence.
You’ll work closely with colleagues across Children’s Disability Social Care, Education, and Health Services to ensure a joined-up, thoughtful approach. While experience in this area is an advantage, we offer full training and ongoing CPD, with regular supervision, appraisals, and development opportunities as part of our wider Council support.
This is a hybrid role, combining home and office-based working. The post covers clients across the county.

Working Hours:
Monday to Thursday: 8:30am–5:00pm
Friday: 8:30am–4:30pm
