Band 7 Clinical Nurse Position – Urgent Community Response
Walsall’s urgent community response model is a well-established service, with a fully integrated domiciliary care system and strong relationships with Social Services and the Acute Sector.
As a Band 7 Clinical Nurse, you will coordinate care and guide patients through various healthcare pathways, ensuring they receive appropriate care across primary and secondary services. This includes preventing unnecessary hospital admissions through a national 2-hour targeted response.

As part of one of our unplanned, front-door services, you will manage a caseload of patients requiring complex case management. Your role will focus on preventing hospital admissions and improving health outcomes for patients over a limited period. You will work autonomously while continuing to monitor and manage your caseload, collaborating with the wider nursing team to ensure optimal patient care within the community.

- You will work as part of a multi-disciplinary, multi-agency team, managing a varied caseload of patients in their homes across Walsall. Under the guidance of the Advanced Clinical Practitioner Lead within the UCR team, your focus will primarily be on older people’s services, helping them remain in their homes with enhanced care.
- Flexibility is essential, as the role requires working across a 7-day period to meet service demands. Collaboration with acute services, WMAS, and GPs will be essential.
