About the Role and the Ideal Candidate
We are seeking experienced Social Workers to join our Adult Social Care team in Sutton, where you will play a key role in helping adults live as independently as possible. As part of the Locality Team, you will undertake the following responsibilities:
Care Act 2014 Assessments: Identify and assess individuals’ care and support needs with a personalized approach.
Care Act 2014 Reviews: Evaluate existing care plans to ensure they continue to meet the individual’s needs and preferences.
Managing Safeguarding Enquiries: Investigate concerns about abuse or neglect, ensuring the safety of vulnerable individuals.
Mental Capacity Assessments: Determine an individual’s capacity to make decisions regarding their care and treatment.
Duty Rota: Participate in the team’s duty system.
DSTS and CHC: Work on these frameworks where appropriate.
Court-related Work: Manage cases that require legal intervention.
Our team is known for creating comprehensive, person-centered care plans that involve various professionals, families, and external agencies to ensure optimal outcomes for our service users.
To be successful in this role, you should have:
A degree in Social Care or a related field and be registered with Social Work England.
Experience in a multidisciplinary team setting.
Knowledge and experience working with complex mental and physical health cases.
The ability to communicate complex social issues clearly to both service users and professionals.
A track record of developing and coordinating social care plans and patient discharge processes.
