About the Role
This is an exciting opportunity to join a pioneering 24/7 Community Mental Health Neighbourhood Centre in the west of York. The service is part of a two-year NHS England-funded pilot designed to transform how mental health support is delivered at a community level.
The hub provides an open-access, person-centred environment where adults can receive timely mental health support without traditional barriers. It has been co-designed with partners and communities to improve accessibility, responsiveness, and recovery-focused care.
You will work within a highly collaborative, multi-disciplinary team including clinicians, social workers, peer support workers, recovery workers, social prescribers, and voluntary sector partners.
Key Responsibilities
In this role, you will provide frontline mental health support within a dynamic and evolving service. Your responsibilities will include:
- Delivering initial mental health assessments for individuals accessing the hub
Providing short-term therapeutic and psychosocial interventions
Offering clinical advice, guidance, and specialist input to the wider hub team
Supporting care planning, signposting, and pathway management
Working across multiple settings including the hub, GP surgeries, community locations, and service users’ homes
Contributing to 24/7 service delivery, including some weekend and out-of-hours work (no night shifts)
Supporting collaborative working across primary care, secondary care, and voluntary organisations
Contributing to teaching, training, and upskilling of partner agencies
Helping develop integrated, recovery-focused mental health pathways
About the Service
Tees Esk and Wear Valleys NHS Foundation Trust delivers mental health and learning disability services across the North of England, working in partnership with local communities to improve wellbeing and access to care.
The York Community Mental Health Hub is designed around co-production principles, ensuring service users, carers, and professionals work together to shape care delivery. It represents a shift toward more flexible, accessible, and preventative mental health services.
About You
This role is suited to a registered mental health professional with strong clinical and collaborative skills.
Essential Requirements:
- Current professional registration with an approved regulatory body
Minimum 18 months post-qualification experience (including completion of preceptorship/competency framework where applicable)
Experience working with individuals experiencing mental health difficulties
Ability to work effectively within a multi-disciplinary team
Knowledge of mental health legislation and safeguarding principles
Strong communication and interpersonal skills
Ability to travel independently across the locality
Desirable Experience:
- Experience in community-based mental health services
Leadership, supervision, or mentoring experience
Involvement in service improvement or quality initiatives
Teaching or training experience
Clinical Knowledge & Skills
You will be expected to demonstrate understanding of:
- Mental Health Act and Mental Capacity Act
Risk assessment and risk management approaches
Care Programme Approach (CPA)
Psychological and recovery-focused models of care
Clinical governance and evidence-based practice
Multi-agency working across health, social care, and voluntary sectors
