Role Overview
Somerset NHS Foundation Trust is expanding its Urgent Care Hub and is seeking a motivated and experienced Clinical Lead (Band 6) to help oversee the coordination of mental health inpatient beds across the region.
This is a dynamic role suited to someone who can confidently balance clinical decision-making with leadership responsibilities, ensuring patients receive the right care at the right time while maintaining efficient service flow.
The Hub operates 7 days a week (9am–5pm) and works at the centre of admission and discharge planning across mental health services.
 What the Service Does
The Urgent Care Hub acts as the central point for all mental health admissions, working closely with inpatient wards, Home Treatment Teams (HTT), local authorities, and third-sector organisations.
Its main goal is to:
- Reduce unnecessary hospital stays
Improve patient flow
Support timely and safe discharges
Ensure care continues effectively within the community
 Key Responsibilities
In this role, you will:
- Coordinate and review all requests for psychiatric admissions, ensuring appropriate bed allocation
Maintain real-time oversight of bed availability and patient movement across services
Work collaboratively with wards, HTTs, and external agencies to reduce delays in discharge
Support safe and effective discharge planning, including family and carer involvement
Supervise and guide Band 4 Clinical Assistants on a daily basis
Ensure accurate documentation using clinical systems (e.g. RiO)
Step in for the Team Manager when required
Contribute to service improvement, audits, and governance activities
 Leadership & Clinical Impact
As a senior clinician, you will:
- Make complex clinical decisions under pressure
Provide mentorship, supervision, and support to junior staff
Contribute to training and development, including student placements
Work within multi-agency settings to deliver coordinated care
Apply frameworks like Triangle of Care to involve families and carers
 Essential Requirements
Qualifications
- Professional qualification in mental health (e.g. RMN, Social Work, Occupational Therapy)
Commitment to ongoing professional development
Experience
- Post-registration experience at Band 5 level or equivalent in mental health services
Experience supervising or managing staff performance
Knowledge & Skills
- Strong understanding of mental health legislation
Knowledge of safeguarding responsibilities
Ability to lead teams and delegate effectively
Strong communication, organisation, and decision-making skills
Confidence working in high-pressure, fast-changing environments

This position may be eligible for the UK Skilled Worker visa, depending on eligibility.
