Job Description

We are looking for an enthusiastic and innovative Senior Support Pharmacist to join our growing Community Services team in Solihull. This role is essential in ensuring the safe, effective, and optimised use of medicines across community and intermediate care settings.

About the Role

As a Senior Support Pharmacist, you will play a vital role in delivering specialist pharmaceutical care within our expanded services, which now include:

  • 40 beds at the Elmdon Unit, Solihull Hospital
  • 15 beds at Swallows Meadow Court

Working closely with the Pharmacy Community Services team, GPs, elderly care consultants, and the multidisciplinary team, you will:

  • Conduct medication reviews and reconciliation to optimise patient outcomes.
  • Provide expert advice on medicine administration, dosing, and side effect management.
  • Support discharge planning and educate patients to ensure a smooth transition from care settings.
  • Contribute to the development of policies, protocols, and training programs to enhance clinical governance and promote the safe use of medicines.
  • Identify and implement service improvements to ensure high-quality pharmaceutical care.

Key Responsibilities

  • Actively participate in multidisciplinary teams, bridging primary, secondary, and tertiary care.
  • Support the delivery of clinical pharmacy services across Solihull’s intermediate care settings.
  • Monitor the effectiveness and safety of prescribed medications, supporting prescribers with evidence-based recommendations.
  • Support and mentor junior pharmacists and trainees, promoting a culture of continuous learning and excellence.

What We’re Looking For

You’ll be a qualified and experienced pharmacist, registered with the General Pharmaceutical Council (GPhC), and passionate about delivering patient-centred care. Ideal candidates will:

  • Demonstrate strong clinical knowledge and problem-solving skills.
  • Have experience in providing pharmaceutical care across diverse care settings.
  • Exhibit excellent communication and teaching abilities to support patients and colleagues.
  • Be proactive, innovative, and committed to driving service improvements.

Qualifications & Experience

Essential:

  • Vocational Master’s degree in pharmacy (or equivalent qualification)
  • Membership of the General Pharmaceutical Council (GPhC)
  • Experience in hospital pharmacy, covering a range of specialties
  • Commitment to high-quality pharmaceutical care and professional development
  • Experience in teaching and presenting to healthcare professionals and patients

Additional Skills:

  • Ability to work under pressure and prioritise effectively
  • Strong interpersonal and communication skills
  • Problem-solving and critical appraisal skills
  • Ability to manage budgets and deliver cost improvement programmes
  • Teaching and mentoring skills
