About the Role
This is a unique opportunity to be part of a pioneering 24/7 Community Mental Health Hub in the west of York. Funded by NHS England as a two-year pilot, the service aims to reshape how mental health support is delivered—making it more accessible, responsive, and centred around the needs of local people.
The Hub operates an open-door approach, offering immediate support to adults seeking help. It brings together a wide range of professionals, including clinicians, peer support workers, social workers, and community partners, working collaboratively to deliver holistic care.
This role offers a high level of autonomy, alongside strong multidisciplinary support, and is ideal for someone who enjoys innovation, collaboration, and service development.
Important: Applicants must already have the right to work in the UK. This role does not meet the salary threshold for visa sponsorship.
What You’ll Be Doing
As a Mental Health Practitioner within the Hub, you will:
- Provide initial assessments for individuals accessing the service
Offer clinical advice, guidance, and signposting to appropriate services
Deliver short-term psychosocial and therapeutic interventions
Work across a variety of settings, including the Hub, GP practices, and patients’ homes
Act as a clinical resource and advisor to the wider multidisciplinary team
Manage your own workload while contributing to shared team priorities
Build strong partnerships with primary care, voluntary organisations, and community services
Support system-wide development, including training and knowledge sharing
The role includes some weekend and out-of-hours work, but does not include night shifts.

Essential Requirements
Qualifications
- Current professional registration with a recognised UK regulatory body
Evidence of ongoing professional development
Minimum 18 months post-qualification experience (including completed preceptorship or competency framework)
Level 2 literacy, numeracy, and IT skills
Experience
- Significant experience working with individuals with mental health needs
Experience working collaboratively with service users, families, and carers
Experience within multidisciplinary teams
Involvement in supervision, mentoring, or supporting learners
Participation in quality improvement initiatives
Knowledge & Skills
- Strong understanding of evidence-based mental health practice
Knowledge of relevant legislation
Awareness of safeguarding responsibilities
Skills in clinical risk assessment, care planning, and decision-making
Understanding of psychological models and therapeutic approaches
Knowledge of clinical governance and service improvement

- Ability to travel across the local area is essential (reasonable adjustments considered)
