We are looking for a dedicated and enthusiastic full-time Band 5 Community Nurse to join our Integrated Care Team in Leytonstone (ICT South).
ICT South is an innovative, community-based service that takes a holistic approach to patient care. The team works in close collaboration with physiotherapists, occupational therapists, district nurses, and community matrons to provide seamless and comprehensive care to the people we serve.

Key Responsibilities:
Support the team, department, and organisation in promoting the Trust’s core values: kindness, respect, and collaboration with communities.
As a Registered Nurse with NHS experience, you will be responsible for managing a caseload, assessing, planning, and delivering patient care. While community experience is desirable, it is not essential.
Act as a mentor for pre-registration students and Band 4 nursing assistants/assistant practitioners.
Work independently and as part of the team, collaborating with other health and social professionals.
Use electronic patient records (RIO), so being computer literate is essential; prior experience in administrative tasks is beneficial.

Probationary Period:
This role is subject to a probationary period. Internal applicants are exempt unless they are currently part of a probationary period or a bank member of staff.
Starting with NELFT:
We prioritize a warm welcome and thorough induction for new starters. All new team members join on the first Monday of each month for a comprehensive two-week induction, which includes mandatory training, systems training, and equipment allocation. You’ll also have opportunities to meet senior leaders and attend engagement and wellbeing sessions. The induction takes place at our head office in Rainham, Essex.
High Cost Area Supplement – London:
This post also includes a High Cost Area Supplement, providing 15% of the basic salary (minimum £4,551, maximum £5,735, pro-rata for part-time staff).
