Tees Esk and Wear Valleys NHS Foundation Trust is seeking an experienced and motivated Physiotherapist to join its Older People’s Mental Health Community Team based in Seaham.

This rewarding role offers the opportunity to support older adults living with complex physical, psychological, and mental health conditions, including individuals experiencing frailty syndrome, dementia, and functional mental health difficulties.

Working as part of a multidisciplinary community mental health team (CMHT), you will help improve quality of life, independence, mobility, and wellbeing for vulnerable service users across the community.

About the Role

The successful candidate will work autonomously to assess, plan, and deliver specialist physiotherapy interventions for older adults with complex needs.

Patients supported by the service may present with:

  • Organic mental health conditions such as dementia
    Functional mental health conditions including depression and anxiety
    Frailty syndrome and mobility concerns
    Multiple long-term physical health conditions
    Psychological and cognitive challenges affecting independence

You will use appropriate assessment tools and evidence-based physiotherapy interventions to support recovery, mobility, function, and safety while working collaboratively within a multidisciplinary environment.

Main Responsibilities

The role will include:

  • Conducting physiotherapy assessments for older adults with complex mental and physical health needs
    Developing and implementing person-centred treatment plans
    Delivering interventions targeting frailty, mobility, strength, and functional independence
    Participating in multidisciplinary team meetings and daily clinical huddles
    Supporting risk assessments and safe discharge planning
    Maintaining accurate clinical documentation and outcome measures
    Contributing to audit, service improvement, and evidence-based practice initiatives
    Working in partnership with carers, families, and healthcare professionals

Candidate Requirements

Essential Qualifications

  • Degree or recognised qualification in Physiotherapy
    Current HCPC registration
    Minimum 12 months post-qualification physiotherapy experience
    IELTS Level 7 (or equivalent) for international applicants where required

Desirable Qualifications

  • Postgraduate training in a relevant specialist area

Essential Knowledge & Experience

Applicants should demonstrate:

  • Understanding of the needs of individuals with mental health conditions or learning disabilities
    Experience delivering physiotherapy interventions independently
    Strong communication and multidisciplinary teamwork skills
    Ability to manage complex clinical presentations

Desirable Experience

  • Experience working with older adults with complex needs
    Community or mental health physiotherapy experience
    Knowledge of clinical risk assessment
    Understanding of evidence-based physiotherapy practice
    Experience managing complex rehabilitation cases
