Role Overview

An opportunity is available for a compassionate and motivated individual to join a Community Mental Health Team supporting older adults in Whitby. This role focuses on helping people remain independent while receiving care in their own homes or residential settings.

You will be part of a multidisciplinary team, working alongside nurses, therapists, and other healthcare professionals to deliver high-quality, person-centred support. The role involves regular contact with patients, families, and carers, requiring strong communication skills and a flexible, understanding approach.

As this is a community-based position, you should be comfortable travelling across a wide geographical area and able to work both independently and as part of a team.

Key Responsibilities

In this role, you will support individuals with mental health needs through a combination of clinical, practical, and emotional care. Your responsibilities will include:

Delivering safe and person-focused care within community settings
Supporting the monitoring of mental and physical health under professional supervision
Assisting with therapeutic, non-medication-based interventions tailored to individual needs
Helping individuals set and work towards personal recovery goals
Working with care homes to review and support residents’ mental health needs
Building strong working relationships across health and social care services
Supporting physical health monitoring, especially during medication adjustments
Carrying out basic clinical tasks such as ECGs or blood sampling (training provided)

Candidate Requirements

Essential Qualifications

• Level 2 Diploma in Health and Social Care (or equivalent)
  Care Certificate
  Level 2 literacy and numeracy (or equivalent)
  Completion of the Trust’s Healthcare Assistant competency framework within 6 months

Experience

• Previous experience working in a care environment (Band 2/3 or similar role)

Skills and Attributes

• Strong communication skills (written and verbal)
  Ability to manage time effectively and prioritise tasks
  Willingness to work independently when required
  Understanding of equality, diversity, and patient dignity

Additional Requirements

• Ability to travel independently to meet service needs

Desirable:

• Experience supporting individuals with mental health or learning disabilities
  Skills in ECG recording or venepuncture
