Overview of the Role
This is a rewarding opportunity for a Community Mental Health Nurse to join a dedicated Memory Service in Redcar. The team focuses on supporting individuals experiencing cognitive difficulties, including dementia, along with their families and carers.
You will be joining at a time of positive change, contributing to improvements in the local dementia care pathway. The service is designed to ensure early assessment, timely diagnosis, and access to appropriate treatment and support, helping individuals maintain independence and quality of life.
For applicants requiring sponsorship, this role may be eligible under the Skilled Worker visa route (subject to meeting criteria)
Your Role in the Team
Working as part of a multidisciplinary team, you will play a key role in delivering compassionate, person-centred care.
Your responsibilities will include:
- Acting as an initial point of contact for individuals referred to the Memory Service
Carrying out comprehensive, holistic memory assessments
Gathering detailed clinical histories while involving families and carers in care planning
Supporting individuals through the memory care pathway with clear guidance and communication
Maintaining accurate, up-to-date clinical records to support safe decision-making
Working collaboratively with colleagues, external services, and partner organisations
Contributing to service development and continuous improvement initiatives
This role combines clinical expertise with strong interpersonal skills, ensuring individuals feel supported throughout their care journey.

Essential Requirements
Qualifications
- Current registration with the Nursing and Midwifery Council (NMC)
Evidence of ongoing professional development
Willingness to work towards mentorship/assessor and clinical supervision qualifications
Experience
- Experience supporting individuals with mental health conditions or learning disabilities
Experience working in person-centred care environments
Ability to work collaboratively with families, carers, and multidisciplinary teams
Knowledge & Skills
Understanding of best practices in mental health and dementia care
Knowledge of key legislation, including:
- Awareness of safeguarding responsibilities
Skills in clinical risk assessment and care planning
Knowledge of medicines management, including administration of injections
Ability to maintain accurate clinical documentation
Additional Information
This role requires travel within the local area; access to a vehicle is essential (reasonable adjustments will be considered)
