About the Opportunity

Derbyshire Healthcare is looking for a compassionate and motivated Registered Mental Health Nurse (RMN) to join the South and Dales Older Adult Community Mental Health Team as a Band 6 Lead Nurse.

This is an excellent opportunity for an experienced mental health professional who wants to develop their clinical leadership skills while making a meaningful difference to older people living with mental health difficulties.

Our community team works with people aged 65 and over who may be experiencing a range of organic or functional mental health conditions. You will become part of a multidisciplinary service where professionals work closely together to provide safe, effective and person-centred care.

Your Role

As Lead Nurse, you will take clinical responsibility for a designated caseload and provide assessment, treatment and ongoing support to service users.

Your responsibilities will include:

– Managing and reviewing a complex caseload.
– Carrying out initial and comprehensive mental health assessments.
– Identifying and managing clinical risks.
– Developing care plans in partnership with service users and other professionals.
– Acting as the team’s duty worker when required.
– Providing appropriate clinical interventions.
– Administering depot medication in line with clinical requirements.
– Attending Care and Treatment planning meetings and wider multidisciplinary discussions.
– Working collaboratively with health, social care and other partner organisations.
– Contributing to the wider development of community mental health services.

There may also be opportunities to contribute to the Care Home Project, supporting care homes within the local area and helping staff respond effectively to the mental health needs of their residents.

As part of the role, you will:

– Support student nurses during their placements.
– Provide supervision and guidance to junior team members.
– Share your clinical knowledge and experience with colleagues.
– Participate in regular clinical and management supervision.
– Take advantage of training and professional development opportunities.

The Older Peoples Division actively encourages staff to expand their knowledge and skills. Training and development opportunities are supported wherever service requirements allow.

You should be able to demonstrate:

– Registration as a Mental Health Nurse (RMN).
– Experience working with people experiencing mental health difficulties.

– Strong communication and interpersonal skills.
– The ability to work effectively within a multidisciplinary team.
– An interest in developing clinical leadership skills.
– A commitment to supporting students and less experienced colleagues.

You should also be comfortable working collaboratively with professionals across health and social care services.
