Role Overview
An exciting opportunity has arisen for an experienced Qualified Nursing Associate (Band 4) to join the Adult Learning Disabilities Physical Health Team across the Tees Valley.
This role focuses on reducing health inequalities and improving physical health outcomes for adults with learning disabilities. You will work closely with individuals, families, and healthcare services to ensure people receive fair access to high-quality care.
 About the Service
The team supports people with learning disabilities by:
- Improving access to healthcare services
Reducing health inequalities
Supporting early intervention and prevention
Working collaboratively across health and social care systems
You will work alongside:
- Primary care networks (PCNs)
Local authority services
Secondary care providers
Community learning disability teams
 Key Responsibilities
In this role, you will:
- Deliver person-centred health support to individuals with learning disabilities
Support health promotion initiatives across the community
Help implement reasonable adjustments to improve access to healthcare
Provide 1:1 support for individuals experiencing changes in physical health
Offer advice, education, and signposting to families, carers, and professionals
Support training and awareness for healthcare staff on learning disability needs
Advocate for individuals to ensure equal access to services
Work in partnership with health and social care organisations

Working within a multi-disciplinary team (MDT)
 Essential Requirements
Qualifications
- Registered Nursing Associate with the Nursing & Midwifery Council (NMC)
Foundation Degree in Nursing Associate practice
GCSEs (Grade A–C) or equivalent in Maths and English
Experience
- Experience working within a multi-disciplinary team
Understanding of the Nursing Associate role within healthcare services
Experience providing and receiving sensitive or complex information
Ability to follow care plans and clinical procedures
Knowledge & Skills
- Understanding of professional standards and the NMC Code
Awareness of safeguarding responsibilities
Knowledge of when to escalate concerns and seek clinical advice
Strong communication skills (verbal, written, and non-verbal)
Ability to prioritise workload and work independently when required
Commitment to reflective practice and continuous learning
