Role Overview
This is a rewarding opportunity for an experienced Learning Disability Nurse (Band 6) to join a well-established Integrated Learning Disability Team. The role focuses on supporting adults with learning disabilities, particularly those with complex needs and behaviours that challenge, helping them live healthier and more independent lives within the community.
You will work as part of a highly collaborative multidisciplinary team, while also managing your own caseload and contributing to service development.
 About the Service
The team operates in partnership with Durham County Council, using integrated systems such as Azeus alongside NHS systems to deliver coordinated care.
The service brings together professionals from multiple disciplines, including:
- Nursing
Social Work
Psychiatry
Psychology
Occupational Therapy
Physiotherapy
Speech and Language Therapy
The aim is to provide holistic, person-centred care in line with current legislation and best practice.
 Key Responsibilities
In this role, you will:
- Manage a complex clinical caseload, including assessment, intervention, and review
Deliver care using trauma-informed and recovery-focused approaches
Act as a care coordinator, ensuring continuity and quality of care
Support individuals, families, and carers through tailored interventions
Provide clinical supervision, mentorship, and guidance to junior staff and students
Contribute to training, service development, and quality improvement initiatives
Maintain accurate clinical records and reports within required timeframes
 Working Pattern
- Standard hours: Monday to Friday, 9am–5pm
Participation in Nurse on Call rota (out-of-hours: 8pm–8am)
 Travel Requirement
The role requires regular travel across the locality. Applicants must be able to travel independently.

 Essential Requirements
Qualifications
- Registered Nurse (RNLD, RMN, or relevant field) with the Nursing & Midwifery Council (NMC)
Evidence of continuing professional development (CPD)
Practice Supervisor/Assessor qualification (or willingness to achieve within 6 months)
Clinical supervision training (or willingness to complete)
Minimum 18 months post-qualification experience, including preceptorship
Experience
- Working with individuals with learning disabilities and complex needs
Delivering care in a multidisciplinary team
Supervising or supporting staff and students
Engaging with families, carers, and partner organisations
Involvement in quality improvement or service development
Knowledge & Skills
You should demonstrate strong understanding of:
- Relevant legislation
Safeguarding principles and practice
The Care Act and its application in community services
Risk assessment and clinical decision-making
Clinical governance and supervision
Psychological models of care and treatment
Safe medicines management, including injections
