Role Overview
An exciting opportunity is available to join the Scarborough Integrated Community Mental Health Team, supporting adults within a community setting. This role is ideal for someone who is passionate about recovery-focused care and wants to make a meaningful difference in people’s everyday lives.
You’ll work both independently and collaboratively, helping individuals manage their mental health while promoting independence and wellbeing within their communities.
 About the Service
The team provides assessment, care coordination, and therapeutic interventions for adults experiencing a range of mental health conditions—from moderate needs to more complex and enduring challenges.
The focus is on:
- Supporting recovery
Encouraging independent living
Working in partnership with families, carers, and local services
 Key Responsibilities
In this role, you will:
- Manage your own caseload of patients within the community
Carry out comprehensive mental health assessments
Develop and deliver person-centred care plans
Act as a care coordinator, ensuring continuity of care
Work closely with families, carers, and support networks
Collaborate with multidisciplinary teams (MDT) and external agencies
Contribute to service development and innovation
This position requires a balance of autonomy, accountability, and teamwork.
 Mobility Requirement
Due to the nature of community work, you must be able to travel across the locality. Adjustments can be considered where appropriate under

Newly qualified nurses (due to register within 3 months) are welcome and will receive a Preceptorship Programme.
 Essential Requirements
Qualifications
- Registration with a recognised professional body (e.g. NMC, HCPC)
Evidence of continuous professional development (CPD)
Willingness to complete teaching/assessing qualifications if not already held
Experience
Experience working with individuals with mental health needs
(Desirable but not essential)
- Community-based experience
Working within multidisciplinary teams
Collaboration with families, carers, and partner organisations
Knowledge & Skills
You should demonstrate understanding of:
- Relevant legislation
Safeguarding principles in practice
Care Programme Approach (CPA)
Risk assessment and clinical decision-making
Clinical governance and supervision
Psychological approaches to care

This role may qualify for the UK Skilled Worker visa, subject to meeting eligibility criteria.
