Role Overview

This is an excellent opportunity for a Community Mental Health Practitioner to join the Scarborough Integrated Community Team, delivering care to adults experiencing a range of mental health difficulties within a community setting.

The service focuses on recovery-oriented, person-centred support, helping individuals live well in their communities through assessment, intervention, and ongoing care coordination.

This post is also suitable for student nurses due to qualify within the next three months. A full preceptorship and structured training programme will be provided to support transition into the role.

Key Responsibilities

Community-Based Clinical Practice

  • Carry out comprehensive mental health assessments in community settings
    Provide care coordination for individuals with moderate to severe and enduring mental health needs
    Deliver evidence-based interventions aligned with recovery-focused practice
    Support service users in developing personalised care and recovery plans

Caseload Management & Care Delivery

  • Manage an allocated caseload of service users independently
    Monitor progress and adjust care plans based on clinical presentation
    Work collaboratively to ensure continuity of care across services

Multidisciplinary Collaboration

  • Work as part of a multi-disciplinary team including nurses, doctors, psychologists, and social care professionals
    Liaise with statutory and voluntary sector organisations
    Support integrated care planning and shared decision-making with families and carers

What We Are Looking For

Essential Requirements

Registered with an appropriate professional regulatory body (e.g. NMC, HCPC, or Social Work England)
Evidence of ongoing Continuing Professional Development (CPD)
Experience working with individuals with mental health conditions

Knowledge of relevant legislation including:

  • https://www.gov.uk/government/publications/mental-capacity-act-code-of-practice
    Understanding of Care Programme Approach (CPA) and recovery-based models
    Skills in risk assessment, safeguarding, and clinical governance
    Competence in literacy, numeracy, and IT (Level 2 or equivalent)

Desirable Experience

Community mental health experience

  • Working in multidisciplinary teams
    Collaboration with families, carers, and external agencies
    Experience in person-centred and therapeutic environments

Training & Development

  • Full preceptorship programme for newly qualified practitioners
    Regular clinical supervision and managerial support
