Role Overview
This is a rewarding opportunity for a Registered Nurse who wants to move into or further develop within community healthcare. Working across Bridgwater and surrounding rural areas, you will deliver care directly in patients’ homes, helping them manage complex conditions while maintaining independence.
The role offers a balance between autonomy and team support. You’ll make clinical decisions in the community while being backed by experienced colleagues and clinical leads. Newly qualified nurses are also encouraged to apply, with structured preceptorship available.
If you’re looking for variety, flexibility, and meaningful patient interaction, this role provides all three.
Working Pattern
- Day shifts typically between 8am–6pm
One twilight shift per week (2pm–10pm)
One in four weekends (with enhanced pay)
No night shifts
Flexible and part-time options available
A valid driving licence and access to a vehicle are essential, as travel between patients’ homes is required. Mileage reimbursement is provided.
Key Responsibilities
Community Clinical Care
- Deliver nursing care to patients in their own homes
Assess, plan, and implement holistic care plans
Support individuals with complex and long-term conditions
Administer medications, including IV therapies
Specialist Skills Development
You will gain and apply a wide range of clinical skills, including:
- Wound care and tissue viability
Palliative and end-of-life care
Diabetes management
Catheter and continence care
PICC line and drain management
Collaboration & Integrated Working
- Work closely with services such as Hospital@Home to prevent hospital admissions
Liaise with multidisciplinary teams to ensure continuity of care
Communicate effectively with patients, families, and carers
Clinical Governance & Safety
- Maintain accurate and timely patient records
Follow safeguarding and patient safety standards
Adhere to infection prevention and medicines management policies
Participate in quality improvement and service development

What We’re Looking For
Essential Qualifications
- Registered Nurse with a valid registration from the Nursing and Midwifery Council
Degree in Adult Nursing (or equivalent qualification)
Skills & Attributes

Ability to work independently and make clinical decisions
Strong communication and teamwork skills
Willingness to develop a wide range of clinical competencies
Additional Requirements
- This role requires a DBS check
