About the Role
This is a rewarding opportunity for an experienced Registered Learning Disability Nurse (RNLD/RNMH) to join an established Integrated Community Learning Disability Team.
You will work within a multi-disciplinary team (MDT) that includes professionals from:
- Social Work
Psychiatry
Psychology
Occupational Therapy
Physiotherapy
Speech and Language Therapy
Administrative and support services
The role focuses on delivering high-quality, person-centred care to adults with learning disabilities and complex behavioural needs, ensuring they receive appropriate support under the Care Act 2014 and established clinical pathways.
 Key Responsibilities
 Clinical Care & Caseload Management
- Manage a complex community caseload independently
Carry out assessments, interventions, and ongoing care planning
Monitor outcomes and adjust care based on individual needs
Deliver trauma-informed and recovery-focused interventions
 Multi-Disciplinary Working
- Work collaboratively with MDT colleagues and external agencies
Support individuals, families, and carers throughout care planning
Ensure coordinated and consistent service delivery
Leadership & Supervision
- Provide clinical supervision and guidance to junior staff and students
Support training and development within the team
Contribute to professional development and service improvement
 Governance & Compliance
- Maintain accurate documentation and reports within required timescales
Work within legal frameworks including:
Mental Health Act
Mental Capacity Act
Safeguarding legislation
Ensure adherence to NMC standards and revalidation requirements

You will be part of a well-established integrated service led in partnership with local authority colleagues. The team works collaboratively to ensure individuals with learning disabilities receive consistent, high-quality community-based care.

About You
 Essential Requirements
- Current NMC registration (RNLD or RMN; RGN where applicable)
Minimum 18 months post-qualification experience
Completion of preceptorship/competency framework
Experience working in multidisciplinary teams
Ability to manage and supervise clinical caseloads
 Knowledge & Skills
- Strong understanding of learning disability nursing practice
Knowledge of safeguarding procedures and risk management
Understanding of clinical governance and evidence-based practice
Awareness of psychological and therapeutic approaches
Experience with clinical medicines management (including injections where appropriate)

 Additional Requirements
Ability to travel independently across the locality
