Healthcare Assistant, Erewash Care Team

We have a fantastic chance for the appropriate candidate to join our unique and specialised end-of-life care team. You will be joining a big and established team of Healthcare Assistants, managed by a District Nurse. This team is a subset of the larger Integrated Community Team, which includes Community Nursing and Community Therapy.
Shifts are divided into early and late shifts, each lasting five hours. Early hours are from 08:00 to 13:00, while late shifts are from 16:00 to 21:00. The team allocates these based on your working hours over a seven-day period, with one in every three weekends. The typical staffing allotment is 4 workers each shift, which means you will collaborate closely with your colleagues to conduct joint visits in addition to working alone for individual visits in the community.
We provide end-of-life care to patients in their own homes in the Erewash area. As a result, you will be needed to drive between visits to our two locations: Ilkeston Community Hospital and Long Eaton Health Centre.

Healthcare Assistant, Erewash Care Team
The primary responsibilities of the job will involve the following:
– Personal care for end-of-life patients living at home.
– Patient manual handling with equipment given in the house.
– Continence management, which includes catheter care.
– Care for the pressure area.
– Medication administration using a Medication Administration Record (MAR) sheet.
– Communicate with all members of the Integrated Community Team, as well as use a laptop to enter patient information into Systmone.
