About the Role

We are inviting applications for the position of Bank Ward Discharge Coordinator, joining our dedicated ward teams at Ludlow or Bishops Castle Community Hospitals.

This role is focused on ensuring the safe and timely discharge of patients, including those with complex needs. You will work closely with multidisciplinary teams to coordinate care, liaise with patients, families, carers, and external healthcare providers, and support the implementation of the SAFER Red to Green approach to patient flow and discharge.

Key Responsibilities

As a Ward Discharge Coordinator, your main responsibilities will include:

  • Coordinate and facilitate patient discharges, ensuring a safe and efficient handover from ward to home or community care.
  • Communicate effectively with patients, families, carers, and external agencies involved in patient discharge.
  • Support the SAFER Red to Green initiative, actively contributing to reducing delayed discharges and optimising patient flow.
  • Work autonomously within the ward environment while collaborating closely with nursing, therapy, and medical staff.
  • Maintain accurate and timely documentation of discharge plans and processes.
  • Prioritise and manage workload effectively, using initiative to address challenges in real-time.

Person Specification

Essential Experience & Skills

  • Experience working in a hospital ward environment.
  • Familiarity with the patient discharge process and related protocols.
  • Experience using hospital IT systems for patient tracking and documentation.
  • Knowledge of the Red to Green methodology for improving patient flow.
  • Excellent verbal and written communication skills.
  • Strong organisational skills, ability to work independently, and use initiative.
