We are looking for an experienced and motivated Cardiac Rehabilitation Nurse to become a key part of our established service. This role is ideal for someone ready to take on a leadership position while continuing to deliver high-quality, patient-focused care.

About the Role

As a senior member of the cardiac rehabilitation team, you will take a leading role in shaping how care is delivered to patients recovering from heart conditions. You’ll combine clinical expertise with leadership skills to ensure every patient receives tailored, evidence-based support.

Your work will span both hospital and community settings, giving you the opportunity to support patients throughout their recovery journey.

What You’ll Be Responsible For

Leading and coordinating cardiac rehabilitation programmes across inpatient and outpatient services
Delivering specialist assessments and developing individualised care plans
Supporting patients recovering from conditions such as heart attacks, revascularisation procedures, and heart failure
Providing education that empowers patients to manage their health confidently
Acting as a clinical leader and mentor, guiding junior staff and students
Working closely with multidisciplinary teams including cardiology, physiotherapy, psychology, and primary care
