This is a rewarding opportunity for an experienced Learning Disability Nurse to join an established Integrated Community Learning Disability Team. You’ll play a key role in supporting adults with learning disabilities and complex needs, helping to improve health, wellbeing, and quality of life through person-centred, evidence-based care.
Working within a multidisciplinary team, you will help deliver coordinated services that place individuals and their families at the centre of decision-making.
About the role
This Band 6 position sits within a collaborative Integrated Learning Disability Service, jointly working with Durham County Council and NHS partners. You will use both council and NHS systems in your daily practice, ensuring seamless coordination of care.
You’ll be part of a highly skilled MDT including:
- Social Workers
Psychiatrists
Psychologists
Physiotherapists
Occupational Therapists
Speech and Language Therapists
Nurses and administrative staff
The team works closely together to deliver safe, high-quality, and holistic care using Care Act principles and TEWV clinical pathways.
Working pattern
Monday to Friday: 9:00am – 5:00pm
Participation in Nurse-on-Call rota (8:00pm – 8:00am, out-of-hours cover)
Travel across County Durham required (own vehicle essential for business use)
Your key responsibilities
Clinical care & caseload management
- Manage a complex caseload of individuals with learning disabilities
Carry out comprehensive assessments and care planning
Deliver evidence-based interventions using trauma-informed and recovery-focused approaches
Monitor and evaluate outcomes of care and treatment
Multidisciplinary working
- Work collaboratively with health, social care, and partner agencies
Contribute to MDT discussions and coordinated care planning
Support safeguarding processes and multi-agency working
Professional accountability
- Maintain accurate clinical records using Durham County Council (Azeus) and NHS systems
Ensure compliance with legal frameworks including the Care Act, Mental Capacity Act, and Mental Health Act
Produce reports and documentation within required timescales
👩🏫 Leadership & supervision
- Provide clinical supervision and support to junior staff and students
Contribute to training and development within the team
Support quality improvement and service development activities
Who we’re looking for
You will be a motivated and compassionate nurse who is passionate about improving outcomes for people with learning disabilities.
Essential requirements:
- Current NMC registration (RNLD, RNMH or equivalent)
Minimum 18 months post-registration experience (including completion of preceptorship)
Experience working with people with learning disabilities or complex needs
Ability to manage a caseload independently
Strong communication and teamwork skills
Willingness to travel across the locality
Desirable experience:
- Community or integrated health and social care settings
Supervising or mentoring students or junior staff
Quality improvement or service development work
Skills and knowledge
You should have a strong understanding of:
- Safeguarding adults and children
Mental Capacity Act and Mental Health Act
Clinical risk assessment and management
Evidence-based practice in learning disability care
