Role Overview
An exciting fixed-term opportunity has become available for a motivated Registered Nurse (Band 5) to join the Hepatology Specialist Nursing Team.
This role focuses on supporting the Community Liver Health Check programme, helping identify liver disease at an earlier stage while also contributing to the wider hepatology service across both community and hospital settings.
The post holder will deliver nurse-led clinics, assist with specialist procedures, and work closely with multidisciplinary teams to ensure patients receive coordinated, high-quality care.
Because the service operates across several community locations, the successful candidate must hold a full UK driving licence and have access to a vehicle. Work-related travel expenses will be reimbursed.

This position forms part of the Humber Health Partnership, a large collaboration between:
- Hull University Teaching Hospitals NHS Trust (HUTH)
- Northern Lincolnshire and Goole NHS Foundation Trust (NLAG)
Together, the partnership provides hospital and community care for more than 1.6 million people and supports research, education, and innovation through close links with Hull York Medical School.

Purpose of the Role
You will support the Community Liver Specialist Nurse in delivering patient-centred hepatology care, particularly for individuals undergoing investigation for suspected liver disease or living with chronic liver conditions.
The role combines community outreach, outpatient services, and inpatient support to ensure continuity of care across the patient pathway.
Key Responsibilities
Clinical Practice
- Deliver nurse-led Fibroscan clinics in community and hospital settings
- Support the Metabolic Dysfunction Associated Steatotic Liver Disease (MASLD) pathway
- Assist with the nurse-led paracentesis (ascitic drain) service
- Provide support to inpatients with liver disease when required
- Contribute to surveillance programmes such as hepatocellular carcinoma monitoring
Community & Outreach Work
- Participate in the Community Liver Health Check programme
- Travel to community clinics to improve early diagnosis and patient access
- Educate patients about liver health, lifestyle changes, and treatment plans
Multidisciplinary Collaboration
- Work closely with consultants, specialist nurses, AHPs, and primary care teams
- Provide advice and education to patients, families, and staff
- Support consistent standards of care across community and hospital services
Governance & Professional Practice
- Deliver care aligned with evidence-based nursing standards
- Maintain accurate documentation and confidentiality
- Participate in audit, quality improvement, and service development
- Adapt flexibly to changing service needs and patient demand

Working Pattern
- 30 hours per week

Person Specification
Essential Qualifications
- Registered Nurse with current Nursing and Midwifery Council (NMC) registration

Desirable
- Additional professional training relevant to hepatology or community nursing
Knowledge & Experience
Essential
- Experience working in a busy clinical environment (placement or employment)
- Understanding of professional nursing standards and patient safety
- Ability to assess, plan, implement, and evaluate patient care
- Strong communication and teamwork skills
- Ability to prioritise workload in changing environments
Desirable
- At least six months post-registration experience
- Experience in community healthcare or chronic disease management
- Familiarity with clinical documentation or data systems
