About the Role
This is an exciting opportunity to be part of a pioneering 24/7 Community Mental Health Neighbourhood Centre in the West of York. The service is a two-year NHS England-funded pilot designed to transform how community mental health care is delivered, with a strong focus on accessibility, collaboration, and person-centred support.
The hub provides an open-door approach for adults seeking mental health support and is built on co-production principles, working closely with individuals, families, and community partners to improve outcomes.
You will join a diverse multi-disciplinary team including clinicians, social workers, peer support workers, recovery workers, social prescribers, and voluntary sector staff.
Your Role in the Team
- As a Mental Health Practitioner, you will be a key clinical presence within the hub, providing assessment, advice, and short-term interventions. You will have autonomy in your daily practice while being supported by an experienced multi-agency team.
- The role involves working across a variety of settings including the hub itself, GP practices, patients’ homes, and other community locations. Some weekend and out-of-hours working is required, but there are no night shifts.
Key Responsibilities
Clinical Assessment & Intervention
- Carry out initial mental health assessments for individuals accessing the hub
Provide brief, focused psychosocial interventions
Offer clinical advice and specialist input to the wider team
Signpost individuals to appropriate services and pathways
Multi-Agency Working
- Work collaboratively with NHS clinicians, social care, and voluntary sector partners
Contribute to shared decision-making within a multi-disciplinary team
Build strong working relationships across primary, secondary, and community services
Care Coordination & Support
- Manage your own caseload and daily workload effectively
Support care planning and pathway management
Deliver interventions in a range of community settings
Service Development & Leadership
- Contribute to the ongoing development of the mental health hub model
Participate in training, teaching, and upskilling of partner organisations
Support transformation projects within adult community mental health services

What We’re Looking For
Essential Requirements
- Current professional registration with a recognised body (e.g. NMC or equivalent)
Evidence of ongoing professional development
Minimum 18 months post-qualification experience
Completion of preceptorship or competency framework (where applicable)
Experience working with people experiencing mental health difficulties
Ability to work effectively in a multi-disciplinary team
Strong communication, literacy, numeracy, and IT skills
Understanding of mental health legislation and clinical risk management
Desirable Experience
- Experience in community mental health settings
Leadership or supervisory experience
Teaching, mentoring, or supporting students
Postgraduate qualification in a relevant field
Professional Knowledge Required
Evidence-based mental health practice
Mental Health Act and Mental Capacity Act understanding
Care Programme Approach (CPA)
Clinical risk assessment and safeguarding
Psychological approaches to care
Clinical governance and quality improvement principles
Working Arrangements
- Some weekend and out-of-hours working required

Must be able to travel independently across the locality
