Nottinghamshire Healthcare NHS Foundation Trust is inviting compassionate, motivated Registered Adult Nurses to join our Community Nursing teams. Our community nurses are at the heart of local care delivery, working closely with patients, families and multidisciplinary colleagues to prevent unnecessary hospital admissions and improve long‑term health outcomes. About the Community Nursing Role
As a Band 5 Community Staff Nurse, you will deliver skilled, evidence‑based nursing care across a diverse caseload. You will work flexibly across home and clinic settings, providing holistic assessments and personalised care plans tailored to each individual’s needs.
You will be supported to grow into an expert community clinician, developing confidence in:
- Wound care and leg ulcer management
- Palliative and end‑of‑life care
- Continence and long‑term condition management
- Medication administration and monitoring
- Supporting carers and families
- No night shifts are required, and a dedicated evening and night service provides additional cover outside core hours.
Key Responsibilities
- Assess, plan, implement and review patient‑centred care plans using a holistic approach
- Deliver safe, effective, and evidence‑based nursing interventions
- Promote independence and reduce avoidable hospital admissions
- Work autonomously while contributing to multidisciplinary team decision‑making
- Build strong therapeutic relationships with patients, carers and colleagues
- Safely manage and administer medication in line with policy
- Delegate appropriately and support junior staff and healthcare assistants
- Develop sound clinical judgement and prioritisation skills in a community setting

Essential Requirements
- Registered Adult Nurse (NMC)
- Full UK driving licence and access to a vehicle for business use (reasonable adjustments considered in line with the Equality Act 2010)
- Strong communication, organisational and interpersonal skills
- Commitment to delivering high‑quality, compassionate community care
