Role Overview
Sherwood Forest Hospitals NHS Foundation Trust is seeking an experienced and motivated Senior Practitioner to join our innovative SFH@Home service, delivering advanced nursing care across Virtual Ward pathways, IV Therapy at Home (OPAT), and Medical Day Case Units.
This role sits at the heart of modern, patient-centred healthcare—supporting people to receive safe, effective treatment in their own homes or ambulatory settings, while reducing unnecessary hospital admissions. You will combine advanced clinical practice with leadership, education, and service coordination, helping shape the future of out‑of‑hospital care.

Key Responsibilities
Advanced Clinical Practice
- Deliver high-quality nursing care across Virtual Ward, OPAT, and Medical Day Case services.
- Undertake comprehensive patient assessments, plan and evaluate care, and respond proactively to clinical deterioration.
- Provide IV therapy at home in line with national guidance and local policies, ensuring patient safety and positive experience.
Leadership & Supervision
- Act as a senior clinical role model, supervising and supporting Band 5 nurses and Clinical Support Workers.
- Contribute to staff competency assessment, clinical supervision, and mentorship of students and junior staff.
- Support day-to-day coordination of services, ensuring efficient use of resources and staff deployment.
Service Development & Quality Improvement
- Participate in the introduction and evaluation of new Virtual Ward and community-based pathways.
- Contribute to audit, service evaluation, and quality improvement initiatives.
- Maintain accurate clinical documentation and ensure compliance with governance, infection prevention, and medicines management standards.
Communication & Partnership Working
- Communicate complex or sensitive information clearly and compassionately with patients, families, and multidisciplinary teams.
- Work collaboratively with acute, community, and primary care colleagues to support seamless care pathways.

Person Specification
Professional Registration & Education
Essential
- Current NMC registration (Level 1)
- Diploma or Degree in Nursing or a health-related discipline
- Relevant post-registration qualification (e.g. teaching and assessing)
- Completion of the Trust IV Therapy Package
Desirable
- Mentorship qualification
- Evidence of ongoing CPD and reflective practice
- Leadership or management training
- Research or audit experience
Clinical Skills & Training
Desirable
- Venepuncture and cannulation
- Aseptic Non-Touch Technique (ANTT)
- CVAD management and line insertion
- Oxygen management
- Lone working competency
Experience
Essential
- Experience working in acute and/or community healthcare settings
- Experience supervising non-registered staff and students
- Ability to manage a clinical area effectively
- Understanding of Virtual Ward and IV Therapy at Home models
- Ability to communicate sensitive information with empathy and professionalism
Desirable
- Involvement in audit, research, or service development projects
- Experience acting as a link nurse or project lead
- Experience providing structured feedback and performance support
