An exciting opportunity is available for a Community Mental Health Nurse to join the Memory Team in Knaresborough, based at Alexander House. This role is ideal for a compassionate, motivated nurse who is committed to improving the lives of people living with memory difficulties, dementia, and cognitive impairment, alongside their families and carers.

You may be an experienced Registered Nurse or newly qualified — if you are due to register within the next three months, we strongly encourage you to apply. Comprehensive induction, training, and a Preceptorship Programme will be provided to support your development in this specialist area of practice.

About the Memory Service

This post sits within a period of positive transformation, aligned with the Trust’s Dementia Pathway refresh and ongoing quality improvement work. The service focuses on delivering timely, accessible, and place-based memory assessments, enabling early diagnosis and access to appropriate treatment, support, and planning.

Your Role and Responsibilities

As a Community Mental Health Nurse within the Memory Team, you will:

  • Act as an initial point of contact for individuals referred to the memory pathway and their families
  • Undertake comprehensive, person-centred memory assessments using a holistic framework
  • Gather and document detailed clinical histories, involving families and carers as partners in care
  • Clearly explain the memory pathway, supporting understanding and reducing anxiety from first contact
  • Maintain accurate, timely clinical records to inform MDT discussions and decision-making
  • Work autonomously while contributing actively to multidisciplinary team meetings
  • Collaborate closely with partner services, community teams, and external agencies
  • Support safeguarding, risk assessment, and clinical governance processes
  • Your work will be central to creating a positive first experience and ensuring safe, consistent, and high-quality care.

Essential Requirements

Qualifications

  • Current registration with the Nursing & Midwifery Council (NMC) as RNMH or RNLD
  • Evidence of Continuing Professional Development
  • Willingness to work towards Practice Assessor/Mentor status
  • Trained Clinical Supervisor (or ability to complete within probation period)

Experience

  • Working with people with mental ill health and/or learning disabilities
  • Delivering person-centred care
  • Collaborative working with families and carers
  • Experience within multidisciplinary teams and partner agencies

Knowledge & Skills

  • Understanding of the Mental Health Act and Mental Capacity Act
  • Knowledge of safeguarding, risk assessment, and clinical governance
  • Confident medicines management, including injectable medications
  • Ability to communicate complex and sensitive information effectively

Travel Requirement

Due to the community-based nature of the role, you must be able to travel independently across the locality and have access to a suitable vehicle for work purposes. Reasonable adjustments will be considered in line with the Equality Act 2010.
