Carers Lead Practitioner Uk Sponsorship Available

April 3, 2024
£29.5 - £34.7 / year

Job Description

We are dedicated about helping our carers and ensuring that their rights and needs are met both inside and outside of the service!

We’re constantly searching for better ways to do things. How can we get the most out of our resources? Creating groups in which our carers would desire to participate. Informing our carers about relevant community resources. These are just a few of the things you could check into if you join us. You’ll have a significant impact on what we do, therefore you’ll be expected to take responsibility of your work. You will have time to become involved in the community. To deliver better today. And to build a better future.

Because we are Bracknell Forest.

What it’s like to work with our team…

You will be part of a joint team financed by both Health and Social Care, based at the Bracknell Community Mental Health Team’s offices. You will be responsible for assisting our carers and doing carer assessments.

Regular supervision, team, and MDT meetings are held to assist you, and your involvement is required.

You will be assisted in carrying out the following duties, responsibilities, and training…

● You will be supported in carrying out a caseload of carers evaluations as necessary by the service.

● We will assist you in adhering to the statutory framework necessary for a Community Mental Health Team, which includes the Mental Health Act 1983, Mental Capacity Act 2005, and the Care Act 2014.

● You will collaborate with carers and their families to create a person-centered assessment of carers’ needs and risks, and from there, you will create care plans that are centred on the carer.

● You will be encouraged to collaborate with all internal and external experts using efficient communication.

The advantages you will gain are…

● An inclusive and social working atmosphere with diverse worker networks.

● We provide a hybrid working paradigm to encourage flexible working around personal and family commitments.

● Generous yearly leave begins at 25 days and increases to 28 days after three years of service (pro rata for part time). With an additional yearly leave buy policy.

● Regular possibilities for growth through internal and external training sessions.

● Internal sports and social club opportunities.

● A comprehensive Employee Assistance Programme (EAP) to support health and wellbeing.

● Memberships to the leisure centre and gym are discounted.

● Supermarket discounts.

● Local government pension plan / life insurance coverage.

What are we looking for?

We are searching for an energetic, confident, and dependable individual who is passionate about assisting carers, with a strong emphasis on meeting their legal rights.
