About the Role

As a Senior Nurse Care Manager (RNLD) / Senior Social Worker (Learning Disability Care Management and Transitions Service), you will play a vital role in empowering adults with learning disabilities to live fulfilling lives and have a genuine say in decisions about their care and support.

This is not only about managing a complex caseload but also about shaping lives through a person-centred approach, recognising the uniqueness, strengths, and aspirations of individuals, while engaging their families, carers, and wider community.

About You

You must be qualified as either:

  • An Approved Mentor/Practice Assessor.
  • A Practice Educator Level 2 (or equivalent), or willing to undertake this training on appointment (funded by us).

Experience and skills required:

  • At least 2 years’ post-qualifying experience.
  • Knowledge of current learning disability practice and multi-disciplinary working, including statutory and voluntary agencies.
  • Strong skills in assessment, planning, intervention, and evaluation of support for people with learning disabilities.
  • Ability to build professional relationships, chair meetings, and represent the council in multi-agency environments.
  • Experience using a person-centred approach, creativity in problem solving, and reflective practice.
  • Leadership skills to motivate, mentor, and supervise staff, with a commitment to professional development for yourself and others.

Key Responsibilities

  • Manage a complex and diverse caseload to safeguard the welfare of individuals, families, and carers.
  • Undertake needs assessments, risk management, support planning, commissioning of services, and evaluation of outcomes.
  • Work collaboratively with health and social care colleagues to improve integration and deliver better services.
  • Mentor and advise team members, promoting staff wellbeing and professional growth.
  • Contribute to a learning culture by sharing knowledge and engaging positively with individuals, families, and carers.
