Are you a compassionate and driven Registered Mental Health Nurse who’s ready to make a real impact in your community? We’re looking for a Mental Health Practitioner to join our supportive and innovative Community Mental Health Service at Somerset NHS Foundation Trust.

In this key role, you’ll work closely with individuals facing severe and long-standing mental health challenges, offering person-centred, evidence-based care that promotes recovery, independence, and well-being. What You’ll Be Doing

  • Manage a varied caseload by providing thorough mental health assessments and delivering tailored psychosocial interventions.
  • Build meaningful, therapeutic relationships with individuals and their support networks, including families and carers.
  • Work in partnership with professionals across primary care, social services, and voluntary organisations to ensure holistic support.
  • Take on a leadership role within the team, guiding and mentoring junior colleagues as agreed with your manager.
  • Recognise and act on safeguarding concerns while managing clinical risk in accordance with trust policies.
  • Develop and maintain individualised recovery plans, including crisis and risk management strategies.
  • Actively participate in team meetings, audits, and service improvement projects.
  • Keep accurate and timely clinical records using the RiO electronic system.

What We’re Looking For

You must be a Registered Mental Health Nurse (RMN) with a current registration and a strong commitment to high-quality care in the community. You should have:

  • Experience working with people who live with complex mental health needs.
  • Knowledge of community health and social care pathways.
  • The ability to manage crises and deliver recovery-focused care plans.
  • A valid UK driving licence and access to a vehicle with business insurance, as travel to home visits and meetings across the local area is required.

Who Should Apply
