About the Role

As part of our Adult Social Care Review Team, you’ll play a key role in reassessing and reviewing care plans for adults recently discharged from hospital and those living in the community. Your work will also involve:

  • Supporting key adult social care projects
  • Engaging with care home and domiciliary care providers facing risk
  • Working closely with colleagues in contracts, commissioning, and quality improvement

This is a diverse and rewarding opportunity that will allow you to use and enhance your skills in a CPD-focused environment. You’ll be expected to deliver outcomes-focused, legally literate social work interventions and assessments, with a particular focus on supporting older adults.

What We’re Looking For

We welcome applications from skilled, committed practitioners who meet the following essential criteria:

  • Registered with Social Work England
  • A recognised Social Work qualification
  • Evidence of Continued Professional Development (CPD)
  • A full UK driving licence and access to a car for work

You’ll also need to complete:

  • An enhanced DBS check
  • Satisfactory references and an Occupational Health assessment
