JOB SUMMARY
Are you a band 6 or an experienced band 5 seeking for a new and interesting opportunity? We are looking for a skilled, motivated, and energetic nurse that is passionate about providing high-quality care and developing new services.
You will be in charge of managing your own clinical caseload of community patients, as well as assisting the HCA/rehabilitation assistants with training, supervision, and joint sessions.
The Rehabilitation and Frailty Team (RAFT) provides daily care in the patient’s home for up to 21 days. The service hours are 8 a.m. to 8 p.m., with a care component available from 7 a.m. to 10 p.m. You will work occasional weekends and bank holidays on a rota.
We constantly prioritise our caseload in order to respond to our service users’ changing daily needs, collaborating closely with other service providers to avoid admission when possible, and referring them to alternative community services such as health and social care. As members of the Rehabilitation and Frailty Team, we treat a wide range of patients, including neurological, orthopaedic, falls, frail, end-of-life, admission avoidance, long-term diseases, and musculoskeletal, which allows staff to get comprehensive expertise across multiple specialties. You will be part of a friendly and approachable team that works well together, with strong support from your senior management team.

NELFT prioritises properly welcoming and inducting new employees into the Trust. All new employees will begin their employment with the Trust on the first Monday of each month and will receive a two-week comprehensive induction that will include obligatory training, systems training, and equipment allocation. As part of the process, new hires will have the opportunity to meet with the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellness, and important procedures. The induction will be held at our headquarters in Rainham, Essex.
Probation Period
This position will be subject to a probation period. Internal applicants are excluded from the probationary term (unless you are currently on probation or are a bank employee).
High Cost Area Supplement – London
This position additionally includes a 5% High Cost Area Supplement (minimum payment of £1,192 and maximum payment of £2,011 per year, pro rata).
The position requires a Registered Nurse (RN12-NMC) with considerable intermediate and community care experience at Band 5/6. The post holder will be in charge of overseeing nursing care for a specific caseload. The postholder shall be held professionally and legally liable for all clinical-related aspects of care. Responsible for patients assigned to them, they will assess, manage, and deliver care to fulfil the complicated needs of those patients who require intermediate and primary care services. The position holder will be able to manage and support their peers, work independently, and be an effective team member, in addition to meeting the needs of pre-registration students. They will be devoted to collaborating, actively assisting, and communicating with other health and social care professionals and agencies. Because of the nature of the service, this employment will require you to be available seven days a week.
