Clinical Practitioner / Advanced Nurse Practitioner
Location: Community & Primary Care Setting

We are seeking a dynamic, experienced, and patient-focused clinician to join our multidisciplinary team as a Clinical Practitioner / Advanced Nurse Practitioner, delivering high-quality care to our patients in both care homes and home settings.
Working within your professional boundaries, you will provide comprehensive care—from initial history taking, clinical assessment, diagnosis, treatment, and follow-up, through to the evaluation of care outcomes. The successful candidate will exercise safe, evidence-based decision-making and demonstrate expert clinical reasoning, with an emphasis on safety netting and continuity of care.
Key Responsibilities
A. Clinical Practice
- Provide first-contact care for patients in care homes and community settings, using advanced assessment, diagnostic, and decision-making skills
- Work collaboratively within a multi-disciplinary team to ensure safe, timely, and effective care
- Deliver advanced nursing practice, triaging and treating patients, and making appropriate referrals to primary or secondary care as needed
- Prescribe and review medications (if an Independent Prescriber) based on clinical need, in accordance with local/national guidelines and evidence-based practice
- Diagnose and manage both acute and chronic conditions using drug and non-drug treatment plans
- Promote health education, disease prevention, and wellbeing through tailored interventions
- Coordinate holistic care planning, ensuring patient and carer involvement
- Document clinical consultations and actions accurately in accordance with NMC guidelines and legal standards
- Participate in the 24-hour on-call rota where necessary for community-based care delivery
B. Professional Responsibilities
- Maintain an active NMC registration and uphold the standards outlined in the NMC Code of Professional Conduct
- Stay up to date with current clinical guidelines, policies, and evidence-based practice
- Participate in significant event analysis, audits, and quality improvement initiatives to evaluate and improve care delivery
- Develop and maintain a Personal Learning Plan; engage in continuous professional development
- Work towards or maintain an Advanced Digital Badge
- Support the development and implementation of clinical pathways and multidisciplinary protocols
- Contribute to service planning and delivery in line with PCN priorities
- Collaborate closely with colleagues across the PCN and wider healthcare system
C. Teaching and Mentorship
- Promote a learning environment for patients, carers, students, and other healthcare professionals
- Support the teaching and supervision of medical students, nursing students, GP registrars, and healthcare assistants
- Share expertise and contribute to the ongoing development of staff within the PCN
Health & Safety / Risk Management
- Comply with all health and safety policies and safe working procedures
- Report incidents in line with the organisation’s reporting systems
- Ensure patient and staff confidentiality is maintained at all times
