JOB SUMMARY

 

As a Nurse at TLC Care, you are responsible for the well-being of our residents at the care home, ensuring that their medical, emotional, and social needs are addressed. You would be expected to lead the care team and assess, design, and evaluate customised resident care while consulting with and including residents, GPs, families, and carers.

To assume management responsibilities for a shift as directed.

Main responsibilities

Care & Care Planning.

  • Report any ill health among residents and make requests for GP/professional visits as needed.
  • Ensure that residents’ care plans are prepared and maintained in collaboration with residents, relatives, and other health care professionals, and in accordance with NMC rules and company policy.
  • Assess all aspects of residents’ care needs, and provide health monitoring and direct nursing care as needed.
  • Assess the social and health-care requirements of new residents, and continue to assess and review all previous residents.
  • Maintain correct records and ensure that each resident within the Care Centre has an individually tailored Care Plan.
  • Administer and document prescription drugs in compliance with the NMC recommendations.
  • To be the designated nurse for a number of residents.

Tissue viability

  • Ensure there are no home-acquired pressure sores by providing a high grade of care.
  • Detail any pressure sores and ensure they are reported to the right people.
  • Ensure that residents who are at risk of pressure sores have a customised care plan.

Nutrition

  • Ensure residents complete a MUST assessment monthly.
  • Ensure that residents at risk have an accompanying care plan that details the interventions required.
  • Ensure that fluid and food consumption charts are available for residents with nutrition or hydration issues.
  • Record unique food preferences in the care plan and make sure they are communicated to the kitchen.
  • Ensure residents who need meals via a PEG have a care plan in place.

Continence

  • Complete continence assessments where necessary.
  • All residents with catheters must have a care plan that details catheter care.

Infection Control

  • Maintain and promote the home’s infection control policy.
  • Observe personnel to ensure they are following conventional universal precautions, washing their hands adequately, and wearing gloves and disposable aprons when dealing with body waste.

Resident Hygiene

  • Ensure that all residents are well-groomed and dressed appropriately, and that care records describe resident choices.

Resident Mobility

  • Ensure that residents are able to move independently whenever possible, and that walking aids are available for resident usage.
  • Check that bedrails are not utilised consistently.
  • Residents at risk of falling must have a care plan specifying the treatments required to reduce the risk.

Dignity and Respect

  • Observe and ensure that residents are always addressed by their preferred name in a courteous and respectful way.
  • Monitor care professionals to ensure they knock on residents’ room doors before entering and that they engage with the patients individually.

Communication:

  • Participate in staff, resident, and relative meetings as required.
  • Establish and sustain effective communication with residents, family, and the multidisciplinary team.
  • Deal efficiently with complaints.

Training and Development:

  • Act as a mentor to student nurses and carers
  • Maintain and develop your professional knowledge and skills. Stay professionally current in all areas of clinical competence.
  • Attend necessary training days/courses, on or off-site, as required.

Health and Safety:

  • Understand and ensure the application of the care home’s health and safety policy, as well as emergency and fire procedures.
  • Promote safe working practices within the care home.

General:

Adhere to all applicable NMC standards and regulations.

  • Keep all records as required by the Registering Authority and the General Manager.

Skills, Knowledge, and Qualifications:

  • NMC Registered Nurse: Valid NMC PIN Number
  • Ability to encourage and delegate successfully.
  • Ability to successfully communicate at all levels.
  • Genuinely interested in dealing with the elderly
  • satisfactory DBS and DBS Adult First Check.
  • Team Player – Capable of working within a multidisciplinary team.
