Are you an experienced nurse seeking a flexible, rewarding position? We have an amazing chance for you to join our district nursing team in Mendip.

We’re looking for dedicated nurses with strong communication skills to join our welcoming, vibrant multidisciplinary care team. You will manage a caseload and collaborate with Primary Care Networks and Neighbourhood Teams to provide high-quality patient care in the out-of-hospital setting.

Our service is available seven days a week, from 8 a.m. to 10 p.m. A valid driver’s licence and access to a car are required.

Main responsibilities, tasks, and abilities required

Community nurses work as a team to provide clients with high-quality, compassionate care. The Community Nurse is an autonomous practitioner who is in charge of doing a comprehensive assessment of both complicated and simple care requirements, developing, implementing, and evaluating treatment, and delegating care as needed to other team members. They will assist the interdisciplinary team in providing high-quality care, allowing individuals to accomplish their person-centred goals.

In the absence of the Team Leader, the Community Nurse will be responsible for the patient’s care route as well as non-clinical activities. They will understand how to deal with complex challenges in their daily job and communicate any concerns to a Team Leader.

The Community Nurse will be able to operate effectively as part of a team and will always act within their area of expertise, knowing who to contact for assistance and support as needed. Community nurses will work within agreed-upon procedures, adhering to the necessary workplace standards, regulations, and protocols, such as those outlined in the NMC Code and Professional standards of practice and behaviour for nurses and midwives.

  • Evidence of continued professional development and having completed post-registration training
