Your Role in Adult Social Care: Hospital Pathways Team
We’re looking for a dedicated Social Worker to join our Hospital Pathways Team. Our mission is simple: to empower adults to live as independently as possible. In this vital role, you’ll conduct comprehensive assessments, skillfully manage complex cases, and consistently deliver high standards of person-centred care.
Our teams are renowned for crafting outstanding care plans that genuinely prioritize the needs and voices of service users. You’ll collaborate closely with various professionals, families, carers, and external agencies to ensure the best possible outcomes. Who We’re Looking For: Our Ideal Candidate
We’re seeking individuals with outstanding communication skills and a compassionate manner who can effectively manage high levels of emotional distress. If you have experience in adult care and a passion for delivering person-centred care, we encourage you to apply! In addition, successful candidates will bring:
- A Qualified Social Worker status with current registration with Social Work England.
- Eligibility to work in the UK.
- At least 2 years of post-qualifying experience in adult services, including assessing and supporting individuals with diverse needs and successful completion of your ASYE.
- Sound knowledge of relevant legislation, including the Care Act 2014 and the Mental Capacity Act 2005, as well as social work theories.
- Strong assessment, outcome-focused care planning, and risk management skills.
- The ability to communicate complex social issues clearly to both patients and professionals.
- The capability to plan and coordinate social care plans and key aspects of patient discharge.
