Role Overview
We are seeking experienced SCPHN HV/SN professionals, ideally with a specialist children’s nursing qualification or equivalent, to join our expanding nursing team supporting Looked After Children within Leeds Community Healthcare NHS Trust. These positions are full time.
In this role, you will be part of a warm, supportive team dedicated to improving the health and wellbeing of children and young people in the care of Leeds City Council. Working alongside families and caregivers, you will help maximise health outcomes and tackle health inequalities.
About the Role
As the postholder, you will take responsibility for managing and monitoring healthcare provision for looked after children and young people aged 0–17. Your work will include both direct and indirect engagement with children, young people, families, and carers to drive positive health outcomes.
The role follows a hybrid working model, with around 50% of your time based at our office and the remainder delivering care and support across the West Yorkshire ICB area.
You will be supporting some of the city’s most deprived children and young people. An understanding of the challenges they face, and the ability to adapt your practice accordingly, is essential.
Effective multi-agency and multidisciplinary working are central to this post. Therefore, you must demonstrate excellent communication skills, strong self-motivation, and a genuine commitment to improving the lives of looked after children and their families.
Day-to-day, you will need strong organisational and planning abilities to manage your own workload and diary effectively.

About You
Applicants should have:
- A minimum of two years post-registration experience.
- Knowledge and experience in looked after children’s health and safeguarding.
