Are you ready to reshape how mental health support is delivered in the community? We are looking for a compassionate and experienced Senior Mental Health Liaison Worker to become a vital part of our team in Somerset.
In this role, you won’t just be managing a caseload; you will be breaking down the walls between primary care (GPs) and secondary mental health services. Your work will ensure that adults facing emotional distress—including older adults and those with complex needs—receive the right support at the right time, without getting lost in the system.

What You Will Be Doing
Your day-to-day will be dynamic and varied. You will act as a clinical leader within neighbourhood teams, championing best practices and ensuring continuity of care.
- Clinical Leadership: You will take the lead on assessing, planning, and implementing care interventions for adults (18+). You will act as the lead professional, coordinating treatment plans that actually work for the individual.
- Expert Prescribing: As an Independent Non-Medical Prescriber, you will manage medication reviews autonomously, adhering to the highest professional standards.
- Collaboration: You will work hand-in-glove with GPs and voluntary sector partners. Your goal is to build strong professional relationships that result in seamless care for patients.
- Mentorship: You will be a source of knowledge for your colleagues, providing clinical supervision, training, and guidance to help the wider team grow.
- Risk Management: You will navigate complex situations with resilience, managing risk and delivering difficult news with empathy and professionalism.
Is This Role for You?
We are looking for someone who combines clinical expertise with a genuine desire to improve patient outcomes.
Essential Requirements:
- Registration: You must be a Registered Nurse or Allied Health Professional with current UK professional registration.
- Prescribing: You must be registered as an Independent Non-Medical Prescriber with your professional body.
- Experience: You have a strong background in managing complex clinical risks and mental health presentations. You are also experienced in supervising and training other staff.
- Mobility: You must have access to private transport to travel within the community.
It would be great if you also have:
- Training in Motivational Interviewing or Relapse Prevention.
- Previous experience working directly with clients in a community setting.
- Personal Safety Training.
