Based at Yeovil District Hospital, you’ll join a supportive and experienced Psychiatric Liaison Team (PLT), working with people who present to acute hospital settings with complex mental health needs. This role provides broad clinical exposure, structured development, and a clear pathway to grow within liaison psychiatry.

Whether you’re new to liaison work or looking to deepen your expertise, you’ll be supported through preceptorship (where applicable) and the Trust’s Psychiatric Liaison development pathway, including protected training time and progression opportunities.

The Role

As a Mental Health Nurse within the Psychiatric Liaison Service, you’ll play a vital role in supporting patients during some of their most vulnerable moments.

You will:

  • Carry out comprehensive mental health assessments in an acute hospital environment
  • Assess and manage risk, developing person-centred care and safety plans
  • Deliver evidence-based interventions tailored to individual needs
  • Provide specialist mental health advice and guidance to acute hospital colleagues
  • Work closely with services such as Home Treatment Teams, Community Mental Health Teams, and social care to ensure continuity of care
  • Support admission avoidance and effective discharge planning
  • Maintain accurate clinical documentation and contribute to audit and service improvement
  • You’ll work rotational shifts, including evenings and weekends, and may occasionally support other Trust locations to meet service needs.

Who We’re Looking For

Essential Requirements

  • Registered Mental Health Nurse (RMN)
  • Current professional registration
  • Significant post-registration experience in Community Mental Health, Home Treatment, or Psychiatric Liaison
  • Confidence in managing risk and recognising when to escalate concerns
  • Ability to travel across the Trust area

Desirable Experience

  • Previous experience within Psychiatric Liaison Services
  • Experience across adult and older adult acute mental health settings
  • Demonstrated motivation and commitment to high-quality patient care
