Job Description: Advanced Practitioner

Position Overview:
The post holder will demonstrate a high level of expertise in providing specialist services, offering advice, education, and support to staff, patients, their families, and carers. Practicing autonomously as an advanced practitioner, the role focuses on delivering patient-centred clinical care within the designated specialty. This will include advanced clinical assessment, examination, diagnosis, and treatment within an agreed scope of practice.

The post holder will contribute to innovative ways of working, focusing on improving efficiency and ensuring a patient-centered approach to care. They will be responsible for the safe treatment, referral, and discharge of patients with undifferentiated or undiagnosed conditions. The role also includes overseeing the ongoing development of clinical practice, contributing to policies, procedures, protocols, and guidelines in collaboration with multidisciplinary colleagues.

Key Responsibilities:

Clinical Care & Assessment:

Direct responsibility for patient assessment, examination, investigation, and diagnosis within the assigned specialty.

Manage a personal caseload, ensuring clinical priorities align with agreed protocols and working practices.

Ensure the safe and timely discharge or transfer of patients, identifying and resolving any barriers to transfer or discharge.

Maintain high standards of documentation, ensuring accurate, complete, and up-to-date patient care information in accordance with Trust standards.

Safeguarding & Handover:

Raise concerns related to safeguarding and seek advice and support as needed.

Ensure effective verbal or written handovers of patients between healthcare professionals.

Clinical Governance & Standards:

Monitor the quality of care provided by the team and all clinical staff.

Identify training needs or skill gaps and escalate to the appropriate clinical lead or manager.

Maintain an advanced practice clinical portfolio for submission as part of the annual review process.

Leadership & Development:

Provide leadership and guidance in the development of clinical practice and standards of care.

Adhere to ACP Governance standards and procedures.

The role is primarily clinical (90%) with 10% dedicated to appraisal, audit, teaching, self-development, and research.

Person Specification:

Essential Qualifications:

Professional healthcare registration with a UK regulatory body (NMC / HCPC / GPC).

Post-graduate qualification in the specific specialty or equivalent experience.

MSc in Advanced Clinical Practice or equivalent (must include evidence of health assessment module at Level 7).

Non-Medical Prescribing Course.

Advanced Life Support Qualification.

Recognised Teaching and Assessing/Mentoring Qualification.

Essential Experience:

Extensive post-registration experience at a senior level in the specialty, including significant leadership experience.

Experience working in a multidisciplinary team within a healthcare service.

Ongoing commitment to Continuing Professional Development (CPD).

Experience teaching in various settings.

Sound understanding of NHS care provision priorities and issues.

Flexibility to work across various shifts as per service needs and the willingness to travel between Trust sites as required.
