Job Description

Are you an experienced Practitioner seeking a new challenge?
Are you passionate about supporting adults to achieve positive outcomes? Do you thrive in dynamic environments? If so, we’d love to hear from you.

We are currently seeking a qualified and experienced Social Work professional to join our Adult Social Care team at Conquest Hospital in Hastings. This full-time role will involve working with adults to promote independence, ensure their safety, and provide the highest standard of care.

As a registered Social Worker with Social Work England, you will need to have a thorough understanding of key legislation, including the Care Act (2014), Mental Capacity Act (2005), Equality legislation, and Deprivation of Liberty Safeguards. You will apply these frameworks in your practice, while also fulfilling the Local Authority’s safeguarding responsibilities to protect and mitigate risks for individuals. Experience with Court of Protection work would be beneficial, though training will be provided for successful candidates.

Our team has strong partnerships with local hospital trusts and community organizations, enabling us to work collaboratively to support safe hospital discharges and reablement. We are committed to the home first model, believing that with the right care, clients can remain in their homes safely and maintain their independence. Together with reablement teams and inpatient rehabilitation providers, we aim to build confidence and achieve positive outcomes for those we serve. We also collaborate with our local Integrated Commissioning Boards (ICBs) to ensure people have access to continuing care funding when needed.

One of the unique aspects of our service is that we manage cases for both inpatients and individuals who have been discharged into the community, requiring Care Act assessments.
