About the Role:
Following a successful transformation in Adult Social Care at Bracknell, we are excited to offer the opportunity for a Senior Social Worker to join our Hospital Team. We are looking for an experienced, motivated individual with at least 5 years of post-qualification experience in statutory social services, typically in a local authority setting. The ideal candidate will have an excellent understanding of best practice, legislation, and a professional “can do” attitude.

About the Team:
Our team works collaboratively with health professionals and other partners to prevent unnecessary hospital admissions and ensure timely, safe discharges. We are committed to ensuring individuals receive the right care, at the right time, and in the right place. With regular multidisciplinary forums, we work together to provide holistic, integrated support for residents, helping them stay connected to their communities and social networks.

Key Responsibilities:

Manage a caseload, prioritizing and organizing workloads with regular supervision and guidance.

Work as part of a multidisciplinary team to connect individuals and their families to community services, enabling them to live independently.

Collaborate with colleagues to ensure people receive the right care in the right setting.

Take a strengths-based approach to support individuals, aiming to reduce reliance on long-term care.

Work within allocated resources as outlined in the scheme of delegation.
