About the Role
Somerset NHS Foundation Trust is seeking an experienced and compassionate Senior Mental Health Liaison Worker to join our Community Mental Health Service. This role offers a genuine opportunity to influence how mental health care is delivered across primary care, secondary services, and community settings.
Working closely with neighbourhood teams and Primary Care Networks, you’ll provide early assessment, timely intervention, and coordinated care for adults experiencing mental and emotional distress — including individuals with complex presentations and older adults with comorbidities.
This is a leadership-focused post, ideal for someone who thrives on collaboration, clinical autonomy, and service development, while keeping people at the heart of care.
What You’ll Be Doing
As a Senior Mental Health Liaison Worker, you will:
- Lead comprehensive mental health assessments and develop evidence-based care plans for adults aged 18+
- Act as the named professional for a defined caseload, coordinating multidisciplinary input
- Provide clinical supervision, professional guidance, and consultation to mental health and voluntary sector colleagues
- Work in close partnership with GPs, Primary Care Networks, and neighbourhood teams to ensure seamless care pathways
- Deliver individual and group-based interventions, with a strong focus on risk assessment and management
- Prescribe and review medication autonomously as an Independent Non-Medical Prescriber, in line with national and professional standards
- Contribute to service development, clinical governance, and quality improvement initiatives
- Maintain accurate clinical records and uphold safeguarding, confidentiality, and equality standards at all times
Who We’re Looking For
Essential Qualifications
- Registered Nurse or Allied Health Professional with current UK professional registration
- Qualified Independent Non-Medical Prescriber
- Ongoing commitment to Continuing Professional Development across mild, moderate, and severe mental health presentations
Desirable Qualifications
- Training in Motivational Interviewing and Relapse Prevention
- Recent, relevant post-registration training
Experience & Skills
Essential
- Substantial experience working with people who have complex mental health needs and risk presentations
- Proven experience in providing clinical supervision
- Experience of delivering training or teaching to other professionals
Desirable
- Experience working with service users in community-based settings
Additional Requirements
- Access to private transport (essential)
- Personal Safety Training (desirable)
