About the Role

We are seeking an experienced and compassionate Senior Mental Health Liaison Worker to join our Community Mental Health Service in Somerset. This is a key leadership role within neighbourhood teams and Primary Care Networks, supporting adults experiencing mental and emotional distress — including individuals with complex needs and older adults with comorbidities.

You will play a pivotal role in reshaping how mental health care is delivered across community, primary care, and voluntary sector settings. By working collaboratively across services, you’ll help ensure people receive the right support at the right time, without unnecessary barriers or fragmentation of care.

This role is ideal for a confident clinician who thrives in integrated environments, values early intervention, and is passionate about improving outcomes through joined-up, person-centred care.

What You’ll Be Doing

  • As a senior practitioner, you will balance direct clinical work, leadership, and service development, including:
  • Leading comprehensive mental health assessments for adults aged 18+, including complex risk presentations
  • Acting as the named lead professional for a defined caseload, coordinating care and treatment pathways
  • Developing, reviewing, and delivering evidence-based care plans in collaboration with service users and carers
  • Providing clinical supervision, guidance, and consultation to colleagues across health and voluntary services
  • Working closely with GPs and neighbourhood teams to strengthen continuity of care
  • Delivering individual and group interventions, with a strong focus on safe and effective risk management
  • Practising as an independent non-medical prescriber, including medication reviews in line with professional standards
  • Contributing to service improvement, clinical governance, and quality initiatives

Who We’re Looking For

Essential Qualifications

  • Registered Nurse or Allied Health Professional with current UK registration
  • Qualified Independent Non-Medical Prescriber registered with the appropriate professional body
  • Ongoing commitment to Continuing Professional Development in mental health practice

Desirable Qualifications

  • Training in Motivational Interviewing and/or Relapse Prevention
  • Evidence of recent, relevant mental health training

Experience & Skills

Essential

  • Significant experience supporting adults with complex mental health and risk presentations
  • Proven experience providing clinical supervision to other professionals
  • Experience delivering teaching or training within mental health settings

Desirable

  • Experience working with service users in community-based settings

Additional Requirements

  • Access to private transport to support community work
  • Personal Safety Training (desirable)
