Job Summary
Somerset NHS Foundation Trust is looking for a Registered Mental Health Nurse (RMN) to join the Mendel Assertive Outreach Team (AOT). This role focuses on delivering high-quality, person-centered care to people with severe and enduring mental health difficulties, supporting recovery and independence.
You will work closely with service users, carers, primary and secondary care services, and voluntary/community partners to provide joined-up care. The role also includes mentoring junior staff and contributing to service development, ensuring the Assertive Outreach Team continues to deliver innovative, community-focused mental health care.
Key Responsibilities
Comprehensive assessments & care planning – Create and regularly review person-centered care plans tailored to individual recovery goals.
Therapeutic interventions – Provide evidence-based psychosocial interventions, supporting service users with daily living, finances, housing, and community engagement.
Caseload management – Plan, implement, and evaluate care for a defined group of service users.
Family & carer support – Engage families and carers in recovery, offering appropriate advice and guidance.
Clinical leadership – Supervise and mentor junior staff and students, contributing to preceptorship and professional development.
Integrated working – Build effective links with inpatient units, CMHTs, HTTs, GPs, and voluntary organizations to ensure continuity of care.
Safe practice – Maintain accurate electronic records, uphold the NMC Code, Trust policies, and the Mental Health Act.
Service improvement – Take part in audits, research, and service development projects to improve the Assertive Outreach model.
Person Specification
Qualifications
Essential:
Registered Mental Health Nurse (NMC registration).
Commitment to ongoing professional development.
Desirable:
Recent relevant training.
Mentorship qualification.
Experience
Essential:
Post-qualification experience in mental health.
Experience in community mental health or Band 6 level.
Care planning experience.
Desirable:
Working with severe and enduring mental illness.
Experience in home-based care and supporting hard-to-engage clients.
Supervising junior staff.
Skills & Competencies
Strong communication skills (written and verbal).
Ability to supervise, mentor, and lead a team.
