About the role
This is a genuinely exciting opportunity to be part of a new, collaborative role developed through community transformation work between TEWV and Tees Valley Primary Care Networks (PCNs).
We are looking for a motivated, compassionate and recovery-focused Nursing Associate who wants to make a meaningful difference to the physical health outcomes of people living with Serious Mental Illness (SMI).
People with SMI often experience significant health inequalities. In this role, you will play a vital part in addressing that gap by ensuring patients receive timely physical health checks and follow-up care — using an assertive outreach and person-centred approach.
Your time will be shared between secondary mental health services and primary care settings, allowing you to work across organisational boundaries and build strong relationships with GP practices and community teams throughout the Tees Valley.
What you’ll be doing
As a Qualified Nursing Associate, you will:
- Work closely with GP practices to identify patients on the SMI register who are overdue physical health checks
- Proactively engage patients using assertive outreach approaches to support attendance and completion of investigations
- Deliver care in a way that is respectful, inclusive and recovery-focused
- Support the delivery of physical health monitoring in line with national guidance
- Work as part of an integrated Community Mental Health Team and Primary Care workforce
- Manage a defined caseload with support from clinical and managerial supervision
- Accurately record care provided and escalate concerns appropriately
- Contribute to reducing health inequalities across the local population
- Your role will align with national NHS priorities, including improving physical health outcomes for people with severe mental illness

- A structured appraisal process to identify learning and development needs

- Support to maintain NMC registration and revalidation

Person specification
Essential qualifications
- Registered Nursing Associate on the NMC register
- Nursing Associate Foundation Degree
- GCSEs (or equivalent) in Maths and English at Grade A–C / Level 2
- Eligibility to work in the UK
Essential experience & knowledge
- Clear understanding of the Nursing Associate role within a multidisciplinary team
- Ability to work within the NMC Code and professional standards
- Understanding of professional boundaries and when to escalate concerns
- Knowledge of NMC revalidation requirements
- Awareness of the importance of following care plans, procedures and policies
Essential skills
- Strong communication skills (verbal, written and non-verbal)
- Ability to organise and prioritise delegated work
- Confidence managing unpredictable workloads and patient contact
- Ability to build therapeutic relationships with patients, families and carers
- Commitment to reflective practice and clinical supervision
- Ability to support learners, including Nursing Associate students and support workers
Desirable
- Experience working in a healthcare or community setting
- Qualification in assessment, supervision or teaching in clinical practice
Travel requirement
Due to the nature of the role, you must be able to travel independently across the locality and have access to a suitable vehicle for business use. Reasonable adjustments will be considered in line with the Equality Act 2010
