Senior Nurse Care Manager (RNLD) / Senior Social Worker – Transitions Team

- Contract: Permanent
- Hours: 37 per week
- Salary: PO3 £41,511 – £44,711 per annum
About the Role
Are you a passionate Registered Learning Disability Nurse (RNLD) or Senior Social Worker ready to make a genuine difference? This is an exciting opportunity to join the Learning Disabilities Care Management and Transitions Service in Leeds—supporting young people as they move into adulthood and helping individuals with learning disabilities live independently and with dignity.
As a Senior Nurse Care Manager or Senior Social Worker, you’ll do more than manage a complex caseload. You’ll take a strengths-based, person-centred approach—championing the voice, rights, and ambitions of people with learning disabilities, working collaboratively with carers, families, and community partners.

About You
You’ll be registered with either:
- The Nursing and Midwifery Council (NMC) as a Registered Learning Disability Nurse
Check NMC registration details - Or Social Work England as a Qualified Social Worker
Check SW England registration
You’ll also:
- Have at least 2 years post-qualification experience
- Be an Approved Mentor/Practice Assessor or Practice Educator Level 2, or willing to undertake training funded by us
- Bring expertise in working with adults with learning disabilities, effective assessment and safeguarding practices, and multi-disciplinary collaboration
You’ll be confident in supervising others, leading meetings, and offering mentorship to staff and students.
Key Responsibilities
- Manage a complex caseload and assess, plan, deliver, and review support using a strengths-based, person-centred framework
- Safeguard individuals by identifying and responding to risk and abuse in line with the Care Act 2014
- Provide direct support, mentoring, and supervision to colleagues, newly qualified staff, and student placements
- Work across multi-agency settings to develop tailored care plans and commission services
- Contribute to service development and policy improvements, sharing learning and innovation with the wider team
