Job Description

We’re offering an exciting opportunity for a Mental Health Wellbeing Practitioner to join our services supporting older adults across the Durham area. This role is ideal for someone who values compassionate, psychologically informed care and wants to work closely with people in their own communities to improve wellbeing, independence, and quality of life.

You’ll work with a high level of professional support, including close case management supervision from an HCPC-registered Applied Psychologist, while also having the autonomy to deliver interventions in community settings.

Your Role

As a Mental Health Wellbeing Practitioner, you will deliver brief, evidence-based, psychologically informed interventions to adults in the community, working closely with multidisciplinary colleagues.

Your work will include:

  • Delivering structured, wellbeing-focused interventions under clinical supervision
  • Supporting collaborative care planning with service users, carers, and MDT colleagues
  • Carrying out risk assessments and contributing to risk management plans
  • Engaging sensitively with individuals experiencing distress, emotional difficulty, or cognitive impairment
  • Gathering and analysing information from service users, families, and clinical records
  • Developing formulations and outcome-focused care plans
  • Setting collaborative, achievable goals with service users

Interventions You’ll Deliver

You’ll work alongside experienced mental health professionals to provide a range of low-intensity interventions, including:

  • Behavioural Activation and Graded Exposure (GOALS programme)
  • Problem-solving approaches
  • Sleep improvement strategies
  • Recognising and managing emotions
  • Guided self-help for bulimia and binge-eating
  • Confidence-building interventions
  • Support with medicines management

What We’re Looking For

Essential Qualifications

Graduate Certificate (Level 6) or Postgraduate Certificate (Level 7)
in the national curriculum for Mental Health and Wellbeing Practitioners
(Adult Specialist Mental Health)

Essential Knowledge & Experience

  • Experience supporting people with mental health needs through supervised clinical practice
  • Experience running groups or structured activities
  • Experience working within a multidisciplinary team
  • Confidence analysing and communicating complex information, verbally and in writing
  • Ability to build empathic, supportive relationships with people in distress and their carers
  • Experience contributing to assessment, formulation, and care planning
  • Awareness of safeguarding, confidentiality, and professional boundaries
  • Ability to prioritise workload in fast-paced or challenging environments
  • Understanding of cultural diversity, inclusivity, and systemic barriers to care
  • Awareness of how social, economic, and environmental factors impact mental health

Desirable

  • Lived experience of mental health difficulties
