About the Role
This is a meaningful opportunity for an experienced and empathetic Mental Health Practitioner to join a Community Mental Health Service supporting adults with complex mental health needs.
The role is open to Registered Mental Health Nurses (RMN), Occupational Therapists, or Social Workers with valid professional registration. You’ll play a key part in delivering person-centred, recovery-focused care while working within a collaborative, multidisciplinary environment.
This position blends clinical practice, leadership responsibilities, and service development, making it ideal for someone looking to grow professionally while making a real difference in people’s lives.
 What You’ll Be Doing
 Clinical Responsibilities
- Carry out thorough mental health assessments, including risk and safeguarding considerations
Deliver evidence-based psychosocial interventions tailored to individual needs
Develop and review Recovery Care Plans and Risk Management Plans
Support individuals during relapse or crisis, promoting stability and recovery
 Caseload & Collaboration
- Manage a defined caseload using a strengths-based and recovery-oriented approach
Build therapeutic relationships with service users and their families
Work closely with carers, GPs, social care teams, voluntary organisations, and other partners
 Leadership & Team Contribution
- Provide supervision and mentorship to colleagues when required
Contribute to team coordination in the absence of the Team Manager
Participate in service development, audits, and quality improvement initiatives
 Skills & Experience Required
Essential
- Professional qualification in Mental Health Nursing, Social Work, or Occupational Therapy
Current registration with the relevant UK regulatory body
Strong experience working in community mental health settings
Proven ability to assess and manage risk, particularly in crisis situations
Experience in care planning, implementation, and evaluation
Desirable
- Lived or professional insight into mental health challenges
 Additional Requirements
Full UK driving licence
Access to a vehicle with business insurance (due to community-based travel)
