About the Role
Rossendale PCN is seeking an experienced Advanced Nurse Practitioner (ANP) to provide maternity cover within its Care Homes and community services, funded under the Additional Roles Reimbursement Scheme (ARRS).
This is a senior, autonomous role for an ANP who is confident managing complex clinical presentations across care homes and for housebound patients. You will work at an advanced level, delivering holistic care from assessment through to diagnosis, treatment, and follow-up, while contributing leadership and clinical expertise across the multidisciplinary team.
As this is an ARRS-funded post, applicants must already hold or be eligible to obtain the Advanced Practice digital badge. This requirement is non-negotiable.
The role involves working across multiple GP practices and care homes within the PCN, therefore access to your own vehicle is essential.
What You’ll Be Doing
In this role, you will provide high-quality, patient-centred care to residents in care homes and patients who are housebound, using advanced clinical knowledge and independent decision-making.
Your responsibilities will include:
- Acting as a first point of clinical contact for care homes and community patients
- Conducting advanced assessments, diagnoses, and management of acute, chronic, and long-term conditions
- Providing continuity of care and supporting admission avoidance where appropriate
- Prescribing independently (where qualified), in line with national guidance and best practice
- Coordinating holistic care plans with patients, families, and carers
- Working collaboratively within the multidisciplinary team (MDT) across primary and secondary care
- Supporting care home staff through clinical advice, education, and guidance
- Maintaining accurate, timely clinical records and safeguarding patient confidentiality
- Escalating concerns promptly when patient safety or deterioration is identified
Professional & Leadership Responsibilities
As an Advanced Nurse Practitioner, you will also:
- Work towards (or maintain) the Advanced Practice digital badge
- Maintain NMC registration and practise in line with the NMC Code
- Engage in reflective practice, audit, and peer review to enhance clinical quality
- Promote evidence-based practice and support service development
- Contribute to PCN clinical pathways, protocols, and audits
- Participate in MDT meetings and PCN forums
- Support education and supervision of learners and colleagues where appropriate
Teaching, Learning & Development
You will help foster a learning culture by:
- Supporting education for care home staff and health professionals
- Contributing to training for students, GP registrars, and nursing colleagues
- Maintaining a Personal Development Plan and ongoing CPD

Person Specification
Essential Experience
- Minimum 5 years post-registration experience
- Extensive experience managing long-term conditions (e.g. asthma, COPD, diabetes, CHD)
- Evidence of ongoing CPD to support advanced practice
- Demonstrated ability to evaluate and reflect on clinical effectiveness and safety
Essential Qualifications
- Registered General Nurse with current NMC registration
- Independent Nurse Prescriber
- Master’s Degree
- Postgraduate qualification at Level 7 or above in a relevant specialist area
- Advanced Practice digital badge (or eligibility via accredited/e-portfolio route)
Desirable
- Teaching or mentoring qualification or experience
- Experience undertaking audit and service evaluation
Knowledge & Skills
You will demonstrate:
- Strong understanding of primary care policy, governance, and quality frameworks
- Knowledge of NICE guidance, National Service Frameworks, and evidence-based practice
- Clear understanding of professional accountability and medico-legal responsibilities
- Excellent communication, leadership, and organisational skills
- Ability to analyse clinical information and adapt care plans accordingly
